Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto's most dynamic neighbourhoods; Flemingdon Park, Fairview and Thorncliffe Park: FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Health Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement. We are primarily funded through the Ministry of Health and Long-Term Care/Ontario Health (formerly: Toronto Central Local Health Integration Network TC-LHIN).

Our approach to community health encompasses the social determinants of health which includes: education, employment, isolation, food security and social supports, and utilizes a community development model to promote health, prevent disease, and strengthen community capacity. The Social Worker will work as part of a community-based interdisciplinary team to provide mental health and psychosocial care, therapeutic counselling, case management and follow up support to clients within the context of a client-centered care model and in accordance with the established standards of professional practice. We are seeking a Social Worker who is committed to strength-based care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to community resources and comprehensive health care.
Employment Terms: Contract Full -Time; evenings and weekend hours can be expected
Contract Duration: until December 31, 2021, with an option to stay as relief staff (as needed)
Salary Range: $34.32 - $40.61 per hour, plus 4% vacation pay in lieu of benefits
Reporting to: Manager, Chronic Disease and Mental Health
Location: Across the three FHC sites – Thorncliffe, Flemingdon and Fairview
