Nurse Practitioner – General Services

Permanent Full Time Position

Quest CHCs General Services currently has an opening for a Full Time Permanent, Nurse Practitioner position. This position is responsible for the provision of primary health care services in accordance with the Standards of Practice for Registered Nurses in the Expanded Class and the Expanded Services for Patients Act, and other clinical activities as set forth by the College of Nurses of Ontario.

This position has a strong emphasis on primary health care, health promotion, disease prevention and community capacity building and works within a harm reduction framework.

The ideal candidates will maximize connections and use of existing community resources by acting as a catalyst who, together with community stakeholders, will facilitate the development and implementation of creative and efficient strategies to maximize health outcomes for our clients, while maximizing the use of community care plan. The candidate must be available to work varying schedules, including 11:30am to 7:30pm on Tuesdays and Thursdays, and some travel may be required at points of service locations.

Qualifications include:

  • A Master’s degree in Nursing required, or a combination of education and experience as sanctioned by the College of Nurses of Ontario.
  • Current and valid registration in the Extended Class with the College of Nurses of Ontario and current registration in good standing with the College of Nurses of Ontario.
  • Experience supporting individuals without a healthcare provider who are also experiencing mental health, addictions or concurrent disorders, street-involved populations (homeless, under-housed, sex trade workers), isolated seniors, sexuality and gender diverse populations (2SLGBTQ+) at risk children, youth and families.
  • Experience working with a trauma-informed approach to service delivery.
  • Experience in program development, implementation, monitoring and evaluation.
  • Experience working with interdisciplinary teams is an asset.
  • Valid Ontario Driver’s License, access to reliable vehicle and a Police Reference Check are required.
