The Southern Network of Specialized Care (SNSC) requires one (1) full-time Health Care Facilitator. Supporting the mandate of the SNSC, the Health Care Facilitator will serve adults with developmental disabilities through leading, developing, facilitating, coordinating, monitoring and updating person centered health care plans with people and caregivers. The Health Care Facilitator will provide intensive evidence based person centered coordination of supports including development of plans and linkages to services for persons with complex health care needs who require a high level of active engagement and ongoing monitoring.

Key Job Responsibilities

  • Achieve positive results for individuals through direct service delivery which provides complex health care support coordination in accordance with the CNSC model.
  • Utilize established linkages within the SNSC & CNSC to assist with exploring local, regional and provincial resources for persons requiring complex health care coordination.
  • Provide cross sector coordination that supports case resolution including service navigation, liaising with cross sectoral service partners, coordinating referrals and support and when necessary advocating for required supports and services.
  • Balance the need to attend to both individual/community safety considerations and promoting self-advocacy and self-determination.
  • Counsel clients and families that are adapting to change or are in crisis and are having difficulty coping, making decisions and problem solving.
  • Develop appropriate strategies for clients who may be resistant to services and who live at risk in the community.
  • Collaborate with the Hamilton Niagara Brant Haldimand Norfolk Developmental Services Ontario to coordinate information sharing required to support the CNSC model.
  • Collaborate with other case management supports and community based health care resources to ensure efficient use of resources in complex support coordination.
  • Complete in a timely and professional manner all documentation requirements.
  • Other duties as assigned.


  • A degree at the undergraduate level in Health Care, Social Service or related field.
  • Five years' experience in a similar role.
  • Knowledge of application of case management principles acquired and preferably maintained through membership in good standing with a regulated college.
  • Advanced knowledge and skills related to health care, developmental disabilities, mental health, family work, and systems issues.
  • Demonstrated ability to work as part of an inter-professional team.
  • Excellent interpersonal, written and verbal communication skills.
  • Demonstrated ability to deal congenially with a variety of people effectively and efficiently with minimal supervision.
  • Demonstrated ability to independently plan and prepare presentations and proposals.
  • Organizational skills and demonstrated ability to handle and respect confidential information with tact and diplomacy.
  • Knowledge of a variety of computer software applications and automated record keeping.
  • A valid province of Ontario's driver's license and access to a reliable and insured automobile.
