As a Care Coordinator, working in collaboration with the patient, physician, caregiver and/or family, you will develop treatment goals, and service and discharge plans in a fiscally responsible manner through face-to-face assessments. In partnership with care partners, you will promote awareness of the services of the LHIN and act as an entry point to the community health care system. In the event of ineligible patients, you will also identify/determine alternative sources of assistance.
