POSITION SUMMARY:

Physiotherapist: Mackenzie Health Outpatient Stroke Program

We are currently seeking a Physiotherapist to work at Mackenzie Health Hospital in Stroke Outpatient Clinic located in Vaughan. The Physiotherapist will provide patient management through all stages of assessment, treatment, consultation and follow-up services to patients. The successful candidate will be familiar working in an outpatient clinic setting.

As the successful candidate, you are a graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists of Ontario. You demonstrate excellent interpersonal, organizational and communication skills, both written and verbal, along with a high level of initiative and adaptability to rapidly changing demands.

RESPONSIBILITIES:

  • As a Physiotherapist (PT) you will be conducting assessments & treatment plans
  • As a PT you will be formulating goals and recommendations
  • Planning and implementing patient centered, goal-directed treatment programs & working with Therapy Assistants
  • Assist in the promotion of physiotherapy services to external referral sources
  • Participating actively in the planning, implementation and evaluation of the program

REQUIREMENTS:

  • · Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high-performance interdisciplinary rehabilitation team
  • · Graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists Ontario
  • · 2 years clinical experience in Stroke/Neurological patient populations is preferred
  • · Experience in assessment and generating treatment plans for patients with stroke/neurological disorders
  • · Knowledge of stroke best practices
  • · Experience with Chronic disease management is a good asset
  • · Sound knowledge of community resources is an asset
  • · NDT training is an asset
  • · Effective interpersonal, organizational and communication skills
  • · Strong customer service focus and excellent problem-solving skills required
  • · A positive work and attendance record is essential
  • · Ability to adapt to new computer applications
  • · CPR + AED certificate
  • · Valid G license required
  • · Criminal background check, Vulnerable sector check and Immunization record (current).
