The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities.

We are looking for a motivated and efficient Pharmacy Manager to ensure smooth daily operations at our Outpatient Pharmacy. To ensure success in this position, you should demonstrate experience and expertise in managing a retail pharmacy and compliance with best pharmacy practice, laws and standards in the outpatient setting. This is a unique and exciting opportunity to work in close proximity with complex patients, caring health care professionals, and hospital staff as we provide services to our growing outpatient patient base and transitional residential beds in the community.

POSITION SUMMARY

Reporting to the Senior Manager, Pharmacy Services, the Manager of Outpatient Pharmacy will lead the Toronto Grace Outpatient Pharmacy team, working in accordance with current Ontario College of Pharmacists standards in procuring, storing and dispensing drug products and drug information for our hospital outpatients, staff, and the community at large. In addition, this position will provide direct clinical support to patients, will promote the safe and appropriate usage of medications, will have fiscal responsibility for the outpatient pharmacy, will provide clinical monitoring of individual patient drug therapies, as well as keeping abreast of current information and changes in official regulations. The Manager of Outpatient Pharmacy assumes professional responsibility for the Outpatient Pharmacy and actively participates in the provision of an essential service for the hospital under the guiding principle of optimizing patient care.

QUALIFICATIONS

Licensed to practice pharmacy in Ontario as Part A Registered Pharmacist (RPh).

Bachelor of Science in Pharmacy (BScPhm) or Doctor of Pharmacy (PharmD).

Knowledge of Provincial and Federal legislation governing all aspects of drug use.

Minimum two years of management experience in a community/retail pharmacy setting.

Commitment to continuous improvement and learning.

Demonstrated resilience to adapt in an ever-changing environment.

Possess initiative in providing high quality, patient-centered pharmaceutical care for a diverse and medically complex patient population.

Strong communication and interpersonal skills in order to effectively facilitate the delivery of pharmacy services in a wider institutional setting including consulting customers, patients and managing employees.

Highly motivated, compassionate individual able to work closely with physicians, nurses, and other Allied Health professionals using an interprofessional approach to providing patient care.

Knowledge of drug distribution systems, including blister packaging, unit dose, smart medication, dispensing devices, e-reminders, etc.

Excellent attendance record.

High degree of accuracy and attention to detail.

Ability to meet bone fide occupational requirements as outlined in the Job Demands Analysis.

MAIN RESPONSIBILITIES

Responsible and accountable for the outpatient pharmacy, including the premises of the pharmacy, along with the equipment, systems, inventory and human resources, ensuring adequate staffing required to protect against and mitigate risks associated with delivery of services.

Works with the Senior Manager of Pharmacy Services in the planning and supporting of pharmacy services in accordance with the Standards of Practice for Pharmacists, the Standards of Operation for Pharmacies and all relevant legislation.

Model leadership behaviours and create a positive work environment where staff are motivated to do their best and strive for continuous improvement, demonstrating respect and support for diversity within and among our patients, families and teams.

Maintain Ontario College of Pharmacists standards of accreditation by developing and maintaining policies and procedures to comply with these standards.

Maintain and protect patient confidentiality in accordance with relevant legislations and organizational patient privacy and confidentiality policy requirements.

Demonstrate patient-centered focus and commitment to the Mission, Vision, Value and goals of the department and the Toronto Grace Health Centre.

Manage the commercial operations of the Outpatient Pharmacy.

Develop and deliver on retail and sales-related performance targets.

Oversee, manage and evaluate the work of technical and professional staff.
