As an Integrated Palliative Care Nurse Practioner you will provide leadership, clinical case consultation and direct clinical care to adult clients and their family/caregivers who require hospice palliative care within the HNHB HCCSS. The NP will support and enhance the capacity of primary care practitioners providing hospice palliative care. The NP will work collaboratively with the palliative care team to reduce avoidable emergency department visits/hospitalizations. Hours of Work: Monday to Friday, 8:30am-4:30pm. Your home branch would be Brant with the expectation that you would provide coverage to both Haldimand Norfolk & Brant areas. This is a 6 month term assignment (subject to extension or termination).
