Detailed Overview

Under the direction of the Manager, Clinical Operations and reporting clinically to the Clinical Practice Leader, Social Work, performs duties such as providing clinical supervision for department Social Workers in designated patient care areas for Renal Services. Provides consultation for social workers and interdisciplinary teams in complex care planning. Coordinates the student placement program and facilitates ongoing education and improvement in quality of clinical Social Work practice. Provides direct clinical services to patients and families.
Responsibilities

  1. Coordinates the day-to-day social work workload to ensure optimal use of resources and efficient use of department staff by methods such as facilitating resolutions to complex situations, providing clinical guidance and advice and collaborating with the Clinical Practice Leader, Social Work as required.
  2. Supervises Social Workers' clinical practice by methods such as clinical consultation, participating in performance evaluations, assisting in planning staff training, development and orientation requirements, organizing and implementing in-services and education programs for staff in collaboration with the Clinical Practice Leader, Social Work.
  3. Provides clinical services to patients as assigned by methods such as assessing and identifying risks and challenges, exploring the emotional psycho-social response to illness, and collaborating with interdisciplinary team to determine and implement optimal treatment planning.
  4. Completes psychosocial assessments, family/social histories, discharge summaries, according to departmental and professional charting standards. Maintains staff charting standards by methods such as reviewing staff charting and charting audits.
  5. Participates on interdisciplinary/interdepartmental and professional committees and in patient conferences by methods such as attending meetings, providing input and communicating needs and concerns related to issues including processes, patient care, quality and workload. Collaborates with members of the interdisciplinary team to facilitate the resolution of issues and advocates for the goals and objectives of the programs in accordance with social work standards and scope of clinical practice.
  6. Participates in the planning of new and/or existing patient programs by methods such as identifying the effectiveness of service delivery in meeting the patient and family social, emotional and cultural needs, responses to illness, hospitalization and coping abilities, making recommendations and assisting the Clinical Practice Leader in implementing and evaluating these programs.
  7. Reviews and develops the department's policies, procedures and quality standards and/or activities related to social work clinical practice by methods such as identifying service needs, promoting quality management activities, evaluating the results in collaboration with the Clinical Practice Leader and maintaining the department manuals and advising staff of changes as required.
  8. Coordinates the supervision of Social Work students by ensuring appropriate matching with field instructors, liaising with the university programs and ensuring the proper documentation is in place, supporting the field instructors in observing the student's patient/family interviews, reviews of documentation and evaluation in accordance with departmental and university standards.
  9. Participates in the recruitment and retention of new staff by methods such as collaborating with the Clinical Practice Leader, Social Work to select candidates, developing interview tools, participating in interviews and providing input to the evaluation of candidates.
  10. Provides education and in-service training to community agencies, patients, families and members of the interdisciplinary team as requested by methods such as researching topics preparing and presenting material and documentation to facilitate learning goals. Attends/ participates in departmental, hospital and/or regional educational programs as required.
  11. Maintains knowledge of current developments in evidence based social work practice by methods such as reviewing relevant literature, consulting with other professionals, attending relevant education programs and recommending new/changed practice standards to the Clinical Practice Leader to enhance the delivery of patient care.
  12. Develops and carries out research activities in consultation with the interdisciplinary team and the department with collaboration with the Clinical Practice Leader, Social Work.
  13. Performs other related duties as assigned.

Qualifications

Education and Experience

Master's Degree in Social Work from an accredited School of Social Work. Minimum of five years professional clinical Social Work experience, including one year's supervisory experience or an equivalent combination of education, training and experience.

Current full registration with the B.C. College of Social Workers.
