Salary range

The salary range for this position is CAD $31.89 - $43.65 / hour

We have an exciting Casual opportunity for a Home Health Licensed Practical Nurse Liasion to join our Regional Home Support Team. In this role, you will be deployed to provide support throughout the Fraser Health region.

Detailed Overview

Performs, implements, plans and evaluates nursing care for their own work assignments at assigned locations within the competency and practice guidelines established by the British Columbia College of Nurses and Midwives (BCCNM), legislative scope of practice and consistent with Fraser Health policies and procedures; acts as a liaison between the acute care facility and community care service to facilitate discharge planning from the acute care facility to the appropriate service in the community.


Responsibilities

  1. Assesses and monitors the client's actual and potential strengths and limitations including physiological, psychological, socio-cultural and spiritual needs on an ongoing basis; facilitates, coordinates and provides support for appropriate services consistent with client care plans.
  2. Provides a supportive environment for the client; contributes to the development and modification of client's plan of care; attends and provides input, feedback and recommendations at interdisciplinary client care planning meetings.
  3. Plans, organizes and establishes client care plan priorities; responds to unexpected events and changing client needs within scope of practice and competency guidelines.
  4. Interviews and screens the client by gathering information from various sources; utilizes various client assessment tools to determine appropriate client care plan and reviews client care plan to determine appropriate service upon discharge such as home support, nursing care and/or residential care, in collaboration with the interdisciplinary team.
  5. Performs care according to identified policies and procedures, technology and professional standards of practice; evaluates client outcomes by utilizing various assessment tools, records observations and reports problems and/or changes to designated staff; documents and updates relevant care and procedures information to ensure client safety and to track client progress.
  6. Participates in discharge planning rounds in the acute care facility and facilities timely discharge of patients with the interdisciplinary team ensuring appropriate resources are in place.
  7. Promotes client involvement in health care decisions by facilitating strategies to increase the client's understanding of the care plan, various nursing procedures and/or interventions; reviews and evaluates teaching strategies to optimize client understanding; revises strategies and reviews feedback from the interdisciplinary team; implements corrective action, as directed.
  8. Completes and maintain complete documentation of client information such as forms, charts, records, statistical information and other related information, as required.
  9. Maintains a safe working environment for the client and others by adhering to established safety policies; reports unsafe conditions and/or malfunctioning equipment.
  10. Participates in quality improvement and program activities to maintain consistent level of care and to ensure that the interdisciplinary team work collaboratively; provides recommendations to the Manager.
  11. Assists in the orientation and education of new staff, students and other members of the interdisciplinary team, in collaboration with the Manager or designate.
  12. Advises acute care staff on whether clients are receiving community care services including the tracking of the number of hours of services provided to the patient awaiting placement and discharge.
  13. Performs other related duties as assigned.

Qualifications

Education and Experience

  • Graduation from an approved program of Practical Nursing, plus one (1) year recent related acute care, residential and/or community-based clinical experience, or an equivalent combination of education, training and experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes, as required.
