Detailed Overview

Under the direction of the Manager, Clinical Operations or designate and in collaboration as a member of a multidisciplinary team; performs assessments, plans, implements and evaluates nursing care. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), and according to agency operating policies and standards and unit specific protocols. Provides client and family-centered care through a trauma-informed approach and evidence informed practice. Encourages knowledge exchange in day-to day activities, empowers clients and natural supports to engage and connect within their communities, and ensures they are supported in navigating the health care system. May provide care to patients through a combination of telephone, virtual video/audio conference and/or remote technology to patients in their homes, in the community including outreach, and virtually.


Responsibilities

  1. Assesses patient/resident/family's actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs.
  2. Provides a supportive environment for patient/resident/family. Promotes patient/resident/family involvement in health care decisions; provides information and opportunity to discuss concerns; respects personal choices.
  3. Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences.
  4. Provides nursing care according to identified policies and procedures, technology and FHA standards and evaluates outcomes. Records observations and reports problems and/or changes to designated staff. When required, monitor patients remotely via approved technology.
  5. Performs admissions, discharges, and transfers of patients/residents according to agency operating policies and procedures.
  6. Documents and updates all relevant care and procedure information to ensure patient/resident safety and comfort. Utilizes available software/hardware applications for data collection and analysis.
  7. Operates and maintains effective functioning equipment related to patient/resident care such as bathtubs, scales and mechanical lifts.
  8. Assists in maintaining a safe and clean environment for patients/residents and staff.
  9. Assists in the orientation of new staff, students and other members of the Health Care Team.
  10. Services may be provided on an outreach basis when required in order to assess and/or engage with patients within their own environment and community.
  11. Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees.
  12. Performs other related duties as assigned.

Qualifications

Education, Training & Experience

Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Graduate of a recognized practical nursing program or equivalent combination of education, training and experience.

One (1) year related clinical experience. Completion of or eligibility to complete Mental Health & Substance Use Competency Education.

Current Class 5 Driver's License and access to personal vehicle for business related purposes, as required.

Skills & Abilities

  • Ability to communicate effectively, both verbally and in writing.
  • Ability to deal with others effectively.
  • Ability to organize work and implement a plan of care.
  • Knowledge and competencies to work at full scope of practice as currently required by the BCCNM, including administering medications.
  • Ability to operate related equipment. including applicable software, hardware and remote/virtual technology applications.
  • Physical ability to carry out the duties of the position.
