About the role:

The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)

is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values

and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures

continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,

understanding and commitment to this care philosophy.

The Nurse Practitioner (NP), Hospital to Home (H2H) will be responsible for the direct patient care of patients who

are referred to the H2H program to help provide transition of care back to the community. One of the primary

responsibilities will be supporting the H2H clinic for the overall processing of new patient referrals as well as

coordinating and facilitating adequate home and community care services of patients at home. The NP works within

the program leadership team that includes the H2H Manager, Operations Coordinator, Patient Flow Coordinators,

and home and community care team members. The NP will also be responsible for providing interim coverage for the

Outpatient Antimicrobial Therapy (OPAT) program when required.

The NP reports primarily to the Manager, H2H in collaboration with the Director, Patient Flow & Partnerships, with a

matrix reporting relationship to the Director, Nursing & Health Professions. The role of the NP integrates the six core

competencies of Advanced Practice Nursing as outlined by the Canadian Nurses Association: direct comprehensive

care, health system optimization, education, research, leadership, and consultation and collaboration. The

percentage of time dedicated to each of these domains is variable, based on the program needs and will evolve with

the program.

EMPLOYMENT REQUIREMENTS:

Education/Experience:

* Unencumbered registration as a Registered Nurse in the Extended Class (EC) with the College of Nurses of

Ontario.

* Master's degree in Nursing preferred.

* Membership in NPAO or RNAO preferred.

* Minimum 3‑5 years recent nurse practitioner experience in an acute care and/or home and community care setting

during past 5 year period.

* Demonstrated commitment to the principles of the Patient and Family Centered Care model.

* Clinical knowledge and expertise in home and community care.

* Knowledge of the research process and ability to apply research to clinical practice.

* Previous adult education and program development, implementation and evaluation experience.

Skills/Abilities:

* Strong leadership ability and highly developed interpersonal skills.

* Ability to manage and work collaboratively within an inter‑professional team.

* Ability to be flexible and adapt positively to constantly changing situations.

* Excellent communication, negotiation and conflict resolution skills.

* Effective facilitation skills: individual and group.

* Strong organizational, critical thinking and problem solving skills.

* High level of initiative and self‑direction.

* Ability to plan, manage and implement change effectively.

* Demonstrated commitment to continuing professional education

* Proficient in computer applications and software such as, Microsoft Office, Powerpoint and Excel.

Salary Range: $68.680 to 76.140
