About the role

1 up to 18 month contract 

Under the direction of the Program Manager, the Clinical Services Case Manager plays a key role in supporting the coordination and delivery of case management services within the clinical services team. This position is responsible for managing incoming referrals, maintaining waitlists, facilitating triage processes, and ensuring clients are connected to appropriate services in a timely, equitable, and client-centered manner.

The Case Manager works collaboratively with internal multidisciplinary teams, external partners, and First Nation communities to support seamless service navigation while maintaining accurate documentation and adherence to organizational policies and procedures.

KEY RESPONSIBILITIES

  • Review and process referrals, ensuring complete and accurate documentation
  • Follow up to obtain missing information and assess service eligibility
  • Maintain and manage waitlists to support equitable and timely access
  • Facilitate triage meetings and communicate decisions to stakeholders
  • Coordinate client appointments and support efficient clinician scheduling
  • Match clients to appropriate services and support smooth service transitions
  • Collaborate with internal teams, external partners, and First Nation communities
  • Advocate for clients to promote timely access to appropriate services
  • Maintain accurate records and track referral and service data
  • Prepare reports and identify trends to support program improvement
  • Deliver presentations and participate in meetings and quality initiatives
  • Support outreach efforts, including travel for telepsychiatry promotion
  • Apply culturally safe, trauma-informed, and client-centered approaches

QUALIFICATIONS

  • Post-secondary education in Social Work, Psychology, Health Sciences, or a related field (or equivalent experience)
  • Experience in case management, intake coordination, or healthcare/mental health services preferred
  • Demonstrated understanding of psychology and psychiatric service delivery
  • Strong organizational and time management skills with the ability to manage multiple priorities
  • Excellent communication, interpersonal, and collaboration skills
  • Experience working with Indigenous communities and understanding of culturally safe practices is an asset
  • Knowledge of trauma-informed care principles
  • Proficiency in documentation, data tracking, and reporting systems
  • Ability to travel throughout district communities to support program delivery, with flexibility to work outside standard hours as required
