As the PHC Team evolves, and based on needs of the population, the position may be required to provide new practices such as nurse-led extended hour clinic services and/or first on-call support. Primary Health Care services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in quality improvement, patient safety, community development, clinical practice redesign, and evaluation.
