About the role

ABOUT THE ROLE The Community Health Representative (CHR) is responsible for the prevention and promotion of maintaining a healthy lifestyle, working with a wide range of community members from pre-pregnant, infants, young children, adults, Elders, and working with families as a whole for best community wellness. The role acts as a liaison between community members and resources for better health and wellbeing, client community advocacy, home visits support, community health awareness and environmental health program support for water testing within the community. KEY RESPONSIBILITIES Assists with organizing health education workshops. Conducts home visits to build relationships with community members and provide information and support. Receives clients and their families at the Fort McKay Health Centre or screening site. Coordinates initial interview. Gathers background information and assists in the completion of various forms and applications. Processes various applications to verify client information, checking data for accuracy and completeness. Assists with providing health promotion and injury prevention services such as prenatal classes, Elders' luncheons, or health fairs. Helps prevent and control the spread of communicable diseases (e.g. promotes immunization and communicates information to manage outbreaks). Monitors clients who have chronic diseases. Conducts home and hospital visits. Carries out health screening programs. Refers clients and families to appropriate resources and services and helps them overcome any access barriers. Acts as a liaison and collaborates with community leaders, agencies, and service providers. Helps implement and evaluate community health plans. Assists with coordinating or facilitating community health initiatives. Helps monitor the safety of the community's water supply. Provides information about personal hygiene, safe food handling, safe water quality, and waste disposal. Participates in the investigation of communicable disease outbreaks and provides information, screening and follow-up. Participates in housing inspections, emergency preparedness and disaster planning, and planning and monitoring special events (e.g. Pow Wows, rodeos). Translates or describes routine medical processes and procedures for clients, or interprets cultural practices and beliefs for health care professionals. Communicates general health, safety, personal hygiene, nutrition and communicable diseases to the community at large. Maintains and forwards statistical information of health care referrals. KEY REQUIREMENTS Two (2) years of experience, one year of which is equivalent to a Community Health Representative. Equivalent combination of education and directly related experience will be considered. High degree of cultural competence as it relates to working with Indigenous people. Knowledge of First Nations culture, traditions and values. Working knowledge of computer applications including basic word processing, email, videoconferencing, and internet access and data entry required. Flexibility to travel and work occasional evenings and weekends as needed.
