About the Opportunity
- Ensures accurate completion of LTCF assessments.
- Prepares reports, records, statistics and other data as needed or requested.
- Participates as a member of the interdisciplinary team in assisting individual clients to meet their assessed needs.
- Provides support to clients and families as appropriate.
- Collaborates with clients and family/friends to meet their program expectations and needs.
- Supports the coordination of new admissions
- Supports ongoing education of employees
- Ensure all audits and inspections are completed as scheduled
- Collaborates with Site Manager and facility team to prepare for licensing inspections
- Prepares for and attends various site meetings such as risk management and family conferences
- Collaborates with Site Manager and staffing coordinator to ensure site staffing levels are maintained.
About You
In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
- Practical Nursing diploma or other applicable education
- Membership in professional association
- Current First Aid & CPR required
- Ability to provide a clear criminal record and vulnerable sector check
- Previous experience working in long-term care or with seniors
- Knowledge of the principles of human behavior and ability to apply program solving skills
- Awareness of how decisions impact budget; knowledge of budgetary limitations
- Ability to gather data, compile information, and prepare reports
- Ability to communicate effectively and efficiently, both verbally and in writing
- Ability to develop and foster teamwork and motivate a group
- Strong computer literacy skills (including word processing, spreadsheet, and database software)
- A passion for the healthcare sector and/or ensuring seniors have access to quality accommodations, services and care
