<h3>Job Description</h3><div>Job Description<div><p>Nurse Practitioner (NP) - Long-Term Care</p><p>Shared Position - Bob Rumball Home for the Deaf & Victoria Village Manor</p><p><br />Location: Ontario</p><p>Position Type: Full-Time (Split Between Two Organizations)</p><p>Reports To: Executive Directors / Directors of Care/Medical Director</p><p>Position Overview</p><p>Bob Rumball Home for the Deaf and Victoria Village Manor are seeking a collaborative and resident-focused Nurse Practitioner (NP) to provide advanced clinical care across both long-term care communities.<br><br>This shared role offers a unique opportunity to serve two mission-driven organizations committed to excellence in seniors' care - one specializing in culturally and linguistically appropriate care for Deaf seniors, and the other rooted in community-based, resident-centered long-term care.<br><br>The Nurse Practitioner will play a key leadership role in improving resident outcomes, reducing avoidable hospital transfers, supporting chronic disease management, and strengthening interdisciplinary collaboration across both homes.</p><p>About the Organizations</p><p>Bob Rumball Home for the Deaf</p><p>The mission of the Bob Rumball Home for the Deaf is to provide exceptional care in a communication-rich environment that enhances the quality of life for each Resident. Our vision is to prevent the isolation of Deaf seniors by providing a unique environment which preserves their culture, language and heritage while also serving the hearing community and those who are underprivileged</p><p>Victoria Village Manor</p><p>Victoria Village is a vibrant seniors' community located in the heart of Barrie that provides an aging-in-place experience. Offering 57 Life Lease suites, 16 Senior Residential Suites, a 128 bed Long Term Care facility, and commercial space. Victoria Village is a unique, mixed use campus that provides personalized care and services for those at every stage of aging..</p><p>Key Responsibilities</p><p>&bull; Conduct comprehensive health assessments and develop care plans</p><p>&bull; Diagnose and treat acute and chronic conditions</p><p>&bull; Prescribe medications and order/interpret diagnostics</p><p>&bull; Provide on-site urgent care response to reduce hospital transfers</p><p>&bull; Lead chronic disease management initiatives</p><p>&bull; Support palliative and end-of-life care planning</p><p>&bull; Participate in interdisciplinary rounds and care conferences</p><p>&bull; Provide clinical leadership and mentorship to nursing staff</p><p>&bull; Support infection prevention and quality improvement programs</p><p>&bull; Maintain accurate documentation in electronic health records</p><p>Qualifications</p><p>&bull; Current registration in good standing with the College of Nurses of Ontario (CNO) as a Nurse Practitioner</p><p>&bull; Primary Health Care NP certification</p><p>&bull; Minimum 2-3 years of clinical experience (long-term care preferred)</p><p>&bull; Strong geriatric assessment and chronic disease management skills</p><p>&bull; Excellent collaboration and leadership abilities</p><p>&bull; Experience working with diverse populations</p><p>Preferred Qualifications</p><p>&bull; Experience in long-term care settings</p><p>&bull; Knowledge of Deaf culture and/or ASL (or willingness to learn)</p><p>&bull; Palliative care experience</p><p>&bull; Wound care expertise</p><p>&bull; Experience with PointClickCare or similar electronic systems</p><p>Compensation & Schedule</p><p>Competitive salary commensurate with experience<br />Flexible scheduling structure to accommodate split-site coverage<br />Professional development support</p><p>Commitment to Equity</p><p>Both Bob Rumball Home for the Deaf and Victoria Village Manor are committed to equity, inclusion, and culturally responsive care.
