Job Description

Position Overview:

Reporting to the Manager, the successful candidate will be motivated, self-directed and a key member of a dynamic interdisciplinary team with defined roles focused on quality client care and flow. The successful candidate will be responsible to provide excellent care to a diverse diabetes patient and family population in a fast-paced setting. The Diabetes Coordinator will provide timely access to Diabetes services at SHN and partner organizations (ie. Community based diabetes programs and services). In this role, the Coordinator aides in ensuring timely efficient access to diabetes care within SHN and the Scarborough Community.

The Diabetes Coordinator will maintain and provide surveillance of the diabetes data required for reporting (both Diabetes Education Center (DEC) and Center for Complex Diabetes Care (CCDC) data, as well as the provision of statistical data related to access and volumes that are required to be reported to Ontario Health and the Ministry of Health. They will act as a liaison in aiding education, care delivery, access to care and services and support coordination of diabetes care needs for clients and their families. The Diabetes Coordinator will also provide clinical support in assuring quality of care and best practice diabetes standards and guidelines are being carried out within the program. The Diabetes Coordinator’s actions are governed by the College of Nurses of Ontario, Professional Standards of Practice, Consent to Treatment Act and the philosophy and values and policies and procedures of Scarborough Health Network.

Requirements

Current registration with the College of Nurses of Ontario Baccalaureate Degree in Nursing - preferred At least 5 years of experience as a Certified Diabetes Educator Certified Diabetes Educator (CDE) certification – required Demonstrated leadership, coordinating skills, teaching and technical ability by leading huddles, initiating new practices and processes Strong decision-making and problem solving skills Current teaching and quality assurance experience required Excellent interpersonal and communication skills required Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation. Utilize protective equipment provided and identify and reports hazards Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community Excellent work performance and attendance
