Position Summary The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Primary Health Nurse II provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals. Qualifications EducationGraduate of approved Registered Nursing education program, BN preferred.Must be a member in good standing with CRNM Skills, Expertise and Experience A minimum of five (5) years’ experience working as a Registered NurseA minimum of three (3) years’ experience working in a community health settingKnowledge and Experience working with Students and/or VolunteersCurrent CPR Certification at Basic Rescuer Level Critical SkillsProven knowledge and ability in critical thinking, decision-making, problem-solving, and crisis management Focus on ResultsDemonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes Alignment with Organizational ValuesCommitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission and values Indigenous UnderstandingKnowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families Interdisciplinary Team, CollaborationProven ability to work within an interdisciplinary team environment with respect, professionalism, cooperation, sensitivity and cultural awareness MCC PhilosophiesStrong knowledge, commitment and demonstrated ability to apply diversity, equity, cultural safety, pro-choice, social determinants of health, population health, harm reduction, trauma-informed and community development principles in all aspects of the work CommunicationExcellent verbal and written communication skills and proficiency in the use of computer systems AssetsStrong working relationships with Community Resources Fluency in additional languages relevant to the client populationExperience working with service specific programs including EMR, eChart, and Accuro All positions are subject to Criminal Record, Child, and Adult Abuse Registry checks.
