The Career.

As a Wellness Lead (RPN) at V!VA Leaside, you will play an important role in delivering professional nursing care and supporting the overall wellness of our Community Members. Through your clinical expertise, compassion, and commitment to exceptional care, you will help ensure Community Members receive safe, high-quality care and service every day.

Reporting to the Wellness Manager, you will provide skilled nursing care, complete assessments, develop Care Plans, administer medications, monitor Community Member health and wellness, and collaborate with families, healthcare professionals, and fellow Team Members to support an exceptional Community Member experience.

In the absence of the Wellness Manager, you will provide guidance and support to non-registered Wellness Team Members to help ensure the continued delivery of safe and effective care.

All care services are delivered in accordance with individualized Care Plans, V!VA policies, professional nursing standards, and applicable legislative and regulatory requirements.

Key Responsibilities

Clinical Care & Community Member Wellness

Administer medications and treatments in accordance with physician orders, Care Plans, organizational policies, and professional nursing standards. Provide professional nursing care and service to Community Members. Conduct Wellness Assessments and monitor Community Member health and wellness. Develop, implement, review, and evaluate individualized Care Plans. Assess and document changes in Community Member physical, cognitive, emotional, and behavioural health. Respond to Community Member emergencies and liaise with Emergency Medical Services when required. Provide treatments and skilled nursing care as required. Assist Community Members with medical documentation, questions, concerns, and healthcare-related needs. Provide health education to Community Members and families.

Care Coordination & Communication

Collaborate with physicians, pharmacists, healthcare professionals, families, and fellow Team Members to support Community Member care. Coordinate and participate in care conferences as required. Receive, transcribe, implement, and communicate prescriber orders in accordance with organizational policies and professional standards. Maintain ongoing knowledge of wellness services available within the Community and direct Community Members and families to appropriate supports and resources. Communicate effectively with families and Substitute Decision Makers regarding Community Member care and wellness needs. Work collaboratively with all departments to support Community Member well-being and service excellence.

Documentation, Quality & Compliance

Maintain accurate, complete, and timely clinical documentation and Community Member health records. Conduct audits as required to support regulatory compliance and quality improvement initiatives. Support compliance with the Retirement Homes Act, RHRA requirements, professional nursing standards, and V!VA policies and procedures. Ensure the safe handling, storage, and documentation of medications, including controlled substances. Implement and monitor Infection Prevention and Control practices. Participate in continuing education, training, and professional development activities. Contribute to quality improvement initiatives and departmental goals.

Team Participation & Accountability

Provide guidance and support to Unregulated Care Providers (UCPs) and Personal Support Workers (PSWs) in the absence of the Wellness Manager. Assist with the orientation and onboarding of new Wellness Team Members. Foster a collaborative and respectful team culture focused on exceptional Community Member care. Demonstrate V!VA's Orange Dot Values in every interaction. Participate in department meetings, committees, Community activities, and daily Making Today Great meetings. Foster positive relationships with Community Members, families, visitors, healthcare partners, and Team Members. Complete additional responsibilities and special projects as assigned.

Qualifications

Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario. Experience working with older adults. Experience in retirement living, long-term care, seniors' care, or a related healthcare environment is considered an asset. Ability to delegate tasks appropriately while working within a collaborative team environment. Strong clinical nursing, assessment, and critical thinking skills. Knowledge of Microsoft Office, including Outlook, Word, and Excel. Experience with electronic health records is considered an asset. Strong communication, customer service, and interpersonal skills. Ability to work independently while contributing to a collaborative team environment. Ability to effectively prioritize responsibilities and adapt to changing situations. Commitment to person-centred care and continuous professional development. Ability to maintain confidentiality and exercise sound professional judgement. Able to effectively communicate in verbal and written English. Valid CPR/First Aid Certification required. Smart Serve Certification is considered an asset. Valid Vulnerable Sector Screening (VSS) required. Ability to safely lift, carry, push, and move up to 50 lbs. Ability to work flexible hours, weekends, and statutory holidays as required. Available for call-ins and emergency coverage when required.
