The Opportunity: This isn't a traditional nursing role. Remember When Healthcare is seeking a healthcare professional who is ready to step into a leadership position where relationship-building, problem-solving, and operational oversight take center stage.

As Integrated Care Manager, you will become a key connection point between clients, families, caregivers, scheduling teams, and recruitment. From the moment a new client reaches out, through onboarding and ongoing support, you'll help ensure they receive an exceptional experience while supporting the internal teams that make care delivery possible.

Requirements The Ideal Candidate: You're an LPN who enjoys people just as much as patient care.

Responsibilities: As Integrated Care Manager, you'll be at the center of the client and employee experience, helping ensure care is delivered seamlessly while supporting the teams that make it happen.

You will: Serve as the primary point of connection between clients, families, caregivers, scheduling, and recruitment teams Guide clients through their journey, from initial inquiry and assessment coordination through onboarding and ongoing support Coordinate and oversee client assessments across Nova Scotia, ensuring a smooth and positive experience from start to finish Review assessment documentation for accuracy and completeness while helping maintain high standards of care Build strong relationships with clients through regular follow-up conversations and proactive support Partner with scheduling and recruitment teams to solve challenges, remove roadblocks, and improve operational efficiency Help navigate and resolve escalated client and caregiver concerns with professionalism, empathy, and sound judgment Lead team meetings, support employee development, and foster a culture of accountability, communication, and continuous improvement Monitor day-to-day operations and proactively address service, scheduling, or client concerns before they become larger issues Support the onboarding and development of nurses and field staff, helping set them up for success Maintain accurate documentation and records while ensuring continuity of care and service excellence Identify opportunities to improve processes, strengthen client satisfaction, and enhance the overall employee experience

Qualifications: We're looking for an experienced LPN who is ready to leverage their clinical background in a leadership-focused, non-traditional nursing role. Active Licensed Practical Nurse (LPN) license in good standing with the appropriate regulatory body Previous leadership, supervisory, care coordination, healthcare operations, or client services experience preferred Experience supporting or training other healthcare professionals is considered an asset Strong technology skills and confidence learning new systems and software Experience managing multiple priorities in a fast-paced environment Valid driver's license and access to a reliable vehicle Willingness to travel occasionally throughout Nova Scotia as required Exceptional communication and relationship-building abilities Strong organizational skills with outstanding attention to detail Confident decision-maker who can balance empathy with accountability Ability to navigate challenging conversations professionally and effectively Compassionate, client-centered approach to care and service Self-motivated, adaptable, and comfortable working independently Positive leader who enjoys supporting, coaching, and developing others

Benefits Competitive salary ranging from approximately $68,000–$75,000 annually, based on relevant experience Comprehensive health and benefits package Paid vacation and sick time Mileage reimbursement for work-related travel Monday–Friday schedule, approximately 9:00am–5:00pm Flexibility for occasional evening assessments or meetings, with schedule adjustments as needed In-office leadership role within a growing healthcare organization Six-month onboarding and probationary period designed to support long-term success Opportunity to play a key leadership role and help shape the future growth of a rapidly expanding healthcare organization
