Is currently hiring for Outreach Worker 1.0 EFT Permanent $25.563 - 29.564 per hour Job Summary The objective of the Outreach Program is to provide support to clients living with HIV, who are living with complex needs in relation to adherence to treatment and engagement in primary care services. The Outreach Program supports clients in Winnipeg through Nine Circles Community Health Centre and the Health Sciences Centre HIV Program site. The Outreach Team works in tandem with the Social Work Team to support clients to be empowered in addressing the impact of the social determinants of health in their lives, and to have a voice in the various systems that affect them. The job is a full time permanent position. The regular workdays for this position will fall between 8:00 a.m. and 8:00 p.m. Mondays through Fridays. Regular hours are 7.75 hours per day (net of breaks). Occasional evenings and weekends shifts may be required. Overtime is not a normal requirement of this position. The position is classified as Group 56, on the CUPE Local 204 salary scale. A comprehensive benefit package is provided as per employee eligibility via HEB Manitoba. Working Conditions: This job, which is located at 705 Broadway, requires the incumbent to be physically capable to complete the majority of their work. Please refer to the Outreach Worker Job Hazard Inventory/Analysis (separate document) for detailed description of working conditions. Outreach Workers are expected to be able to physically meet the working conditions as described in the Job Hazard Inventory/Analysis. Outreach Workers must be willing to provide outreach services in a variety of settings including a primary care clinic site, outreach settings including community settings, shelters, encampments and client’s homes. A Community College certificate in Social Services, or health related discipline combined with a minimum of three years related experience; OR A combination of health-related education, along with extensive experience in working with people with diverse needs and challenges as it relates to the social determinants of health, with direct experience in community-based health and social service programs
Knowledge, Skills, and Experience
Extensive personal and/or professional experience working with diverse populations who are reflective of Nine Circles clients Knowledge of addictions, mental health, sexually transmitted infections including HIV and Hepatitis C Demonstrated ability to work in a multi-disciplinary team setting Commitment to and knowledge of community-based health care Excellent communication and interpersonal skills Experience with computers Experience in documentation in medical chart
Required Licenses and Registration
Possession of a valid Manitoba Class 5 drivers’ license Access to a reliable vehicle Clean drivers’ abstract provided upon hire and as required per Nine Circles policy Criminal records, child abuse and vulnerable sector checks provided upon hire and as required by Nine Circles policy
