Position Overview:
Under the direction of the Supportive Housing Manager, the Case Manager acts as clinical lead and one of the senior practitioners of the Supportive Housing 24- hour care setting that is consistent with the vision, mission, and core values established by Dilico Anishinabek Family care. The Case Manager will be primarily responsible for engagement, stabilization with a strong focus on transition and aftercare planning. Responsibilities will be to provide clinical consultation and guidance to the Supportive Housing program team and to ensure continued integration with other internal programs and community partners. Incorporating cultural aspects into the stabilization plan and supportive housing environment as well as case conferencing with external partners for a successful transition out of the program and with First Nations will also be a part of this role.
Qualifications and Requirements:
- A minimum requirement of an HBSW or related degree complimented by significant experience and demonstrated skill in related work will be considered. An MSW is preferred.
- a minimum three (3) or more years related experience utilizing a variety of treatment modalities in the field of Mental Health & Addictions Services particularly relating to Anishinabek clients.
- experience in supervisory or leadership role preferred
- experience in a residential setting would be an asset.
- Experience relating to working in a variety of Mental Health & Addictions programs in the youth/family service system.
- Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.
- Significant demonstrated experience in Adult
Hours of Work: 32.5 hours per week
