Care Assessment Coordinator (RPN Required)

Mission:
The Care Assessment Coordinator is responsible for completing clinical assessments for prospective residents in the community, at home and hospital settings. Working closely with Lifestyle Consultants, this role supports new business development by ensuring timely, accurate assessments and helping guide families through the transition to retirement living. This position does not provide on-site nursing care.

Key Outcomes:

  • Timely completion of clinical assessments for prospective residents
  • Increased conversions and occupancy through strong LC partnership
  • Positive experience for families navigating care decisions

Key Responsibilities:

  • Conduct clinical assessments for prospective residents in hospitals, homes, and the community
  • Partner closely with Lifestyle Consultants to support the sales process and new resident transitions
  • Provide same-day or timely assessments to support hospital discharges and urgent inquiries
  • Act as a clinical resource to help match residents to the appropriate community
  • Build and maintain relationships with hospital staff, discharge planners, and community partners
  • Travel within the region as required; flexible hours based on business needs
  • Maintain accurate documentation and assessment records

Qualifications:

  • Registered Practical Nurse (RPN) in good standing
  • Experience in geriatrics, retirement living, or community care preferred
  • Strong assessment, communication, and interpersonal skills
  • Self-motivated, organized, and comfortable working independently
  • Passion for supporting seniors and their families

Salary: $70,000 - $80,000
