Salary Range: $38.83 – $46.78 / Hour

CORE RESPONSIBILITIES OF THE POSITION
AS A KEY MEMBER OF THE TEAM THE INTAKE SOCIAL WORKER WILL:
- Work within the social work scope of practice
- Implement appropriate strategies by working with the inter-professional team using a client centered approach, providing intake, case management, and brief therapy services to clients
- Contribute to chart documentation as per college and weCHC standards; participate in chart review and case conferences
- Conduct intake assessments and develop individualized treatment plans.
- Deliver brief counselling/therapy for clients with mild to moderate anxiety, depression, and trauma and manage case loads effectively.
- Provide therapeutic interventions: supportive counselling, psychoeducation, crisis management, and evidence-based modalities.
- Monitor progress and document outcomes using EMR.
- Make referrals to other community services as appropriate to support client’s goals.
- Assist clients and community partners to build support networks
- Manage discharge plans, referrals, and post-discharge follow-ups.
- Act as a resource person/client advocate to Centre staff, clients and community partners by compiling information on community resources
- Facilitate access to community resources by liaising with community groups
- Ensure clients receive community support, health, social and recreational services to which they are entitled by acting as a client advocate
- Collaborating with community partners to support population health from a social work perspective
- Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.
SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED
- Minimum of 1-2 years recent experience in counselling/therapy, with preference to experience in a community health setting
- Proficiency in current assessment, and brief counseling techniques for working with individuals, families and the community.
- Additional supervised clinical training an asset
- Adherence to the Code of Ethics and Standards of Practice of Social Work
- Demonstrated ability to work effectively in an inter-professional team environment
- Strong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skills
- Clean driver's abstract, current valid Class "G" Driver's license and reliable transportation to support travel across Windsor-Essex County and the province as required
- Bilingual (English/French) language skills an asset.
EDUCATION, TRAINING AND CERTIFICATE REQUIREMENTS
- Graduate degree in Masters of Social Work (MSW) is required
- Member in good standing and current registration with the Ontario College of Social Workers and Social Service Workers required.
- Completion of a Police Clearance and Vulnerable Sector Check prior to commencing employment.
