About the role
The East Wellington Family Health Team (EWFHT) offers the opportunity to join an innovative,
community-based primary health care team serving the Erin and Rockwood area. We are looking for a
confident, enthusiastic and skilled team player comfortable working within a fast paced, rapidly
changing, and dynamic health care environment. We are currently recruiting for:

Summary
Dedicated and collaborative Nurse Practioner with experience providing comprehensive primary care
within a Family Health Team. Skilled in health assessment, diagnosis, treatment planning, chronic
disease management, preventive care, and patient education across the lifespan. Works closely with
interdisciplinary team members to support timely access to care, improve patient outcomes, and
promote health and wellness through evidence-based practice, screening, immunizations, and
management of complex chronic conditions. The Nurse Practioner must possess valid first aid and CPR
certifications and strong sense of ethics and the ability to handle sensitive or private information with
tact and discretion.
Requirements
- Current certificate of competence as a Registered Nurse with Extended Class privileges with the
College of Nurses of Ontario.
- Experience as a primary healthcare nurse practitioner is preferred
- Proficiency and experience working in a computerized environment
- Knowledgeable regarding the nursing process and its application, including the assessment,
planning, implementation, and evaluation of nursing care.
- Demonstrated ability to assist patients, family members or other clients with concern and
empathy; respecting their confidentiality and privacy and communicating in a courteous and
respectful manner.
- High degree of resourcefulness, flexibility, and adaptability.
- Commitment to staff and patient privacy
- Must be thoroughly familiar with laws, regulations, and guidelines applicable to primary care
and clinical work
Familiarity with East Wellington and the spectrum of health and social services available
- Class G Ontario driver?s license, access to a car, and the ability and willingness to drive between
clinics and, otherwise, as is occasionally necessary
- Proof of up-to-date immunization, CPR/AED Level-HCP, N95 mask fit test, and vulnerable sector
check

Pay: $71,376.00-$75,286.00 per year
