Nurse Practitioner - Mental Health and Addictions Acute and Transitions Prince County Hospital, Summerside, Prince Edward Island

The Nurse Practitioner manages a patient caseload as an integral member of a Health Care Team. The team is composed of a variety of health care providers and team composition may vary dependent on identified needs. The Nurse Practitioner works collaboratively with all health care providers. Direct client services provided by the Nurse Practitioner include client medical assessment, diagnosis and treatment of minor illness, health promotion and chronic disease prevention and management. Works collaboratively with the Most Responsible Provider (MRP) in the Prince County Hospital Mental Health Unit. The Nurse Practitioner is the MRP for the Transition’s Addictions Unit. The Nurse Practitioner supports extended care housing in Charlottetown. The Nurse Practitioner supports clients in Extended Care Housing and the Strength Program by providing regularly scheduled clinics.

Duties will include: perform comprehensive, focused health assessments including a health history and physical examination using a holistic framework and multiple assessment techniques relevant to a client’s stage of development, culture, health and health problems; analyse client symptoms and assessment data by applying the principles of pathophysiology, pathogenesis, epidemiology, demographics and relevant research; determine the need, order, and interpret the results of relevant screening and diagnostic tests; discuss with and involve clients in decision making regarding their health, and in taking responsibility for their health; triage and direct clients and use a collaborative approach at all times; monitor, adjust, and document the outcomes of drug therapy, interventions and treatment plans; provide direct guidance and counselling on health promotion and illness prevention strategies in accordance with client health status and needs; promote and participate in an interdisciplinary provider model; collaborate with interdisciplinary teams and community members to identify client population health needs and resources, and provide input as requested for health programs; use a nursing framework, and theoretical and scientific foundations to guide practice; assess, identify and communicate knowledge deficits and opportunities for sharing of skills and information within Nursing and other health disciplines; other duties as required.

Minimum Qualifications: Graduation from an approved and recognized nurse practitioner program. Eligible for licensure as a Registered Nurse with the endorsement as a Nurse Practitioner with the Association of Registered Nurses of PEI. Maintain CPR certification. Extensive nursing experience in a combination of hospital and community settings. Critical thinking proficiency to assess, analyze, diagnose, select treatment, evaluate, and refer patients with common acute or chronic health conditions, or for health promotion / illness prevention. Knowledge of practice setting programs and the community it serves, as well as current grasp of evidence-based approaches and epidemiology issues pertinent to the catchment area and available resources. Demonstrated ability to work effectively in an interdisciplinary environment with excellent collaborative, team building and consultative skills. Excellent communication (verbal and written), facilitation and interpersonal skills in dealing with clients, families, community groups, professional colleagues, and referral partners. Thorough knowledge and proficiency in medical and nursing physical assessment and treatment methods. Strong analytical, problem-solving and decision-making abilities. Must be available to work flexible hours and shifts such as evenings, nights and weekends as required. The successful applicant must meet minimal productivity requirement; demonstrated ability to be self directed. Must have access to reliable transportation. The successful applicant must have a good previous work and attendance record.

Other Qualifications: Previous experience in a mental health setting preferred. Additional relevant education and experience will be considered an asset. The successful applicant must provide a satisfactory Criminal Records Check prior to beginning employment.

External candidates may be eligible to receive a $8,000 or $18,000 Nurse Practitioner Incentive, plus a $5,000 Rural or $8,000 Student Loan Incentive depending on location of employment. To be eligible for recruitment incentives candidates must hold a 1.0 FTE, this can be a full-time role or a combination of part-time positions. Relocation Assistance may be available to part or full-time candidates.

Salary Range: $57.94 - $65.59 per hour Bi-Weekly Hours: 75.0 hours bi-weekly / Days / Evenings / Nights / Weekends when required / Rotating shifts Posting Id: 172047
