About the role
Stay At Home Nursing Inc. is seeking a compassionate, detail-oriented Care Designer to lead the planning and coordination of personalized home care services for clients across our service region. As a key clinical team member, the Care Designer plays a critical role in conducting in-depth assessments, developing individualized care plans, and ensuring that care delivery aligns with client needs, preferences, and health goals. This role bridges clinical expertise with client advocacy to ensure quality outcomes and satisfaction.
Key Responsibilities:
Client Care and Care Design
- Conduct in-home assessments and build individualized care plans for each client, reviewing and updating every 8–12 weeks
- Design care that reflects not just clinical need but the full picture of who each client is — their preferences, their routines, their goals
- Conduct supervisory visits to assess care delivery quality, field staff–client compatibility, and to maintain trusted relationships with clients and families
- Act as the primary point of contact for clients and families on care questions, concerns, and planning — building the kind of trust that drives continuity and referrals
Field Staff Leadership
- Lead, mentor, and develop the PSW and nursing field staff within your care portfolio
- Support onboarding integration for new field hires — making sure they are prepared for their specific clients, not just paperwork-compliant
- Provide coaching, performance guidance, and, where necessary, formal disciplinary support in line with company policy
- Deliver or coordinate monthly education for your field team — keeping clinical skills current and care standards high
Scheduling and Operations
- Collaborate with the Scheduling Specialist to ensure your client portfolio has consistent, quality coverage
- Anticipate staffing needs and flag emerging gaps before they become crises
- Contribute scheduling intelligence — client preferences, caregiver–client fit, acuity signals — so that scheduling decisions are grounded in care reality, not just availability
Compliance and Documentation
- Maintain accurate, timely care plan documentation in AlayaCare
- Ensure your care portfolio meets all provincial and national regulatory requirements
- Handle confidential client and employee information with discipline and discretion
Qualifications:
Required
Strong Asset
RN or RPN with active CNO registration
Experience in homecare or community nursing
Bachelor’s Degree or Nursing Diploma
Previous supervisory or team lead experience
1–2 years clinical experience
Familiarity with AlayaCare or similar platforms
Valid Ontario driver’s licence and reliable transportation
Experience with care planning documentation
Strong interpersonal and communication skills
Bilingual (English / French)
