What We Do:
Since 1973, The Alex has been a community-based, not-for-profit organization providing integrated primary health care, community support services, and housing-focused programs to individuals and families who experience barriers to accessing traditional health, housing, and social service systems. Services are grounded in principles of health equity, low-barrier access, trauma-informed practice, cultural safety, harm reduction, and person-centered care.
The Mobile Care Clinic is a flexible outreach service that provides low-barrier supports including basic wound care, hygiene supplies, peer support, and harm reduction resources, with referral pathways to additional services as needed.
Under the direction of the Program Manager and Mobile Medical Lead, The Mobile Health Nurse Practitioner (NP) will work to their full scope of practice to provide comprehensive medical and social care to vulnerable clients in Calgary, including individuals with multiple co-morbidities. The Mobile Health NP primarily works on a mobile care unit (sprinter van) at various community sites. This Mobile Care Clinic expands our existing mobile fleet, and the NP role will be instrumental in developing care models and playing a significant role in the provision of collaborative community-based care.
This is a permanent, part-time or full-time position (22.5 – 37.5 hours per week).
What you will do:
- Provide care on the Mobile Care Clinic in collaboration with a Peer Support worker in an outreach capacity. Some services may also be delivered at the Alex Community and Youth Health Centers as needed.
- Deliver episodic primary care to clients in community settings.
- Conduct advanced assessments, order and interpret diagnostic tests, and develop treatment plans.
- Prescribe pharmaceuticals, medical devices, and other therapies.
- Perform procedures, interventions, and treatments (including wound care).
- Provide guidance to members of the multidisciplinary team.
- Adhere to all health and safety regulations.
- Collect data and support evaluation processes.
- Provide support in program design and development.
- Foster a team culture that promotes health and safety and a harassment free environment.
- Join Mosaic PCN upon hire.
- Other duties as assigned based on the evolving needs of the program.
What you bring:
- Active license with the College of Registered Nurses of Alberta, Family /All ages registration is preferred.
- Completion of an approved master’s degree/Nurse Practitioner program, valid BLS provider certification, and triplicate prescribing authority.
- Knowledge, skills, and abilities to work with complex individuals experiencing addictions, mental health concerns and psychosocial complexity including homelessness.
- Strong communication skills and ability to adapt communication style to meet the needs of everyone.
- Comfort working with harm reduction, strength-based, and trauma-informed care approaches, along with an understanding of anti-oppressive practice.
- Ability to set boundaries while effectively building relationships with individuals who may be hard to engage.
- A high level of initiative, flexibility and professionalism.
- Experience, or willingness to expand experience, in addiction medicine and harm reduction, including starting and maintaining opioid replacement therapy. (e.g. Suboxone, Sublocade, Methadone and Naltrexone).
- Experience and interest in sexual health care is an asset.
- Ability to work collaboratively within an interdisciplinary team model and innovative outreach service delivery model.
- Experience with community outreach and crisis intervention is an asset.
- Knowledge of social services and non-profit services in Calgary is an asset.
- Minimum of 21 years of age and three (3) years of driving experience is required to drive an Alex fleet vehicle.
- A Class 5 non-GDL driver’s license, a clean 3-year driver’s abstract, and a personal vehicle that can be used for work purposes.
- Basic Life Support (BLS) within 30 days of hire date; certification must be valid for a minimum of 6 months upon hire.
- A satisfactory vulnerable sector criminal record clearance and child welfare intervention check are required.
- Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action is preferred.
Work Environment and Hours:
- Outreach on the Mobile Care Clinic van as well as at The Alex Youth Health Centre (2840 2nd Ave SE)
- 0.6 to 1.0 FTE (22.5-37.5 hours/week)
- Standard Hours are 8:30am – 4:30pm, Monday to Friday. Some flexibility is required, including the ability to work earlier hours or occasional evening and weekend shifts.
- An extended period of orientation and mentorship will be provided, tailored to your experience and comfort. Enthusiastic new graduate NPs are welcome to apply.
- Extended periods of sitting, standing, bending, and walking.
- Involves physical action requiring the ability to bend/crouch and reach above shoulder height.
- Outreach or home visits, some work in clinic.
- A positive team environment that encourages collaboration and continuous learning.
- Occasional driving may be required as part of job duties.
Driving on Behalf of The Alex
This position requires the use of a personal vehicle for work-related purposes and driving an Alex fleet vehicle. Successful candidates must show proof of a class 5 driver's license, a personal vehicle, and a clean 3-year drivers abstract. Candidates must consent to the possibility of an annual driver's abstract review to meet our insurance provider’s requirements. Driving a fleet vehicle at The Alex requires that staff are at least 21 years of age with a minimum of three (3) years’ driving experience. Mileage will be reimbursed in alignment with the CRA guidelines.
