Working collaboratively with the interdisciplinary team the HCA provides care in alignment with documented care plans and assigned or delegated tasks, delivering assistance with activities of daily living, monitoring and reporting changes in health status, and supporting the physical, emotional, psychosocial and behavioural well being of clients.
