KEY FUNCTION:
Complete screening tools to assist with minimization and identification of length of stay. Promote and educate on the Home First Philosophy. Collect and analyze data to determine outcomes.

REPORTING:
Under the general direction of the Manager, Utilization, Care Transitions, & ALC Leading Practices.

DUTIES:

  1. Engage patients in intial Home First Philosophy conversation.

  2. Meet with patients to complete screening tools used to assess risk for an extended length of stay.

  3. Determine priority of focus in order to achieve an estimated date of discharge (i.e. clinical, social, functional).

  4. In collaboration with a regulated health professional, review completed screening tool(s), assess results and make recommendations on service required based on score.

  5. Act as a point of contact for departmental staff, other departments, programs, and community stakeholders as required.

  6. Attend daily meetings (e.g. Emergency department huddles, bed rounds , bullet rounds, etc.) and be aware of daily bed pressures.

  7. Communicate across care team and engage with community partners effectively.

  8. Develop strong relationships with physicians and interdisciplinary team members in the Emergency department (ED) and across the organization.

  9. Document score and action taken/required on appropriate form and attach to the patient chart.

  10. Provide support for patients, caregivers and informal supporters being served by the organization and community agencies.

  11. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.

  12. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.

  13. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.

  14. Educate and promote health, safety and wellness in the work place.

  15. Represent the department or program on various committees and in meetings as required.

  16. Perform other duties as required.

QUALIFICATIONS

EDUCATION AND TRAINING:

  1. Minimum of a Bachelor’s Degree in health related field or equivalent.

  2. Training in Lean methodologies is preferred.

  3. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

EXPERIENCE:

  1. Minimum of two (2) years’ experience working in a within a health care environment.

  2. Experience working with or knowledge of assessment tools (i.e. InterRAI, Barthel Index, Blaylock, etc.) is preferred.

  3. Experience working with the elderly and their care givers/informal supporters is preferred.

KNOWLEDGE/SKILLS/ABILITIES:

  3. Demonstrated excellent presentation skills.

  4. Demonstrated knowledge of Medical Terminology.

  5. Demonstrated excellent judgment and proven analytical skills.

  6. Demonstrated ability to mentor students and support personnel.

  7. Demonstrated knowledge of current trends in general health care and community resources.

  8. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.

  9. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.

  10. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.

  11. Demonstrated superior interpersonal and communication skills, both written and verbal.

  12. Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.

  13. Demonstrated discretion and maturity when handling confidential information.

  14. Demonstrated commitment to the safety of co-workers and patients.
