Job Description: Reporting to the Manager, Home TPN Clinic, the Home Total Parenteral Nutrition Patient Navigator is responsible for the assessment of patients to identify eligibility, learning needs, education planning and implementation, evaluation, coordination of care and follow-up of Home Total Parenteral Nutrition patients within Saskatchewan. The incumbent collaborates with multidisciplinary team members and liaises with health professionals and agencies within the SHA to facilitate high-quality care for the Home TPN patient.
Human Resources Exemption: No
Education
- Bachelor of Science in Nursing (BScN)
Experience
- One (1) year of experience in adult education or a teaching role
- Requires experience in program/resource development related to needs assessment
- Requires experience in planning, program design, implementation, and evaluation
- Minimum 3 years’ experience with TPN and central venous access devices
Competencies
- Computer skills
Knowledge & Abilities
- Demonstrates effective leadership and team building skill and cooperation.
- Knowledge in time management
- Demonstrates efficient use of resources
- Demonstrates TPN expertise and maintains it through professional development
- Ability to integrate relevant evidence based best practice as std. performance
- Able to transfer info to patients/families on topics essential to health care
- Knowledge in principles of adult education
- Ability to integrate and evaluate pertinent data to problem-solve effectively
- Demonstrates care for clients and families, respecting cultures & preferences.
- Supports client advocacy and responds to social justice concerns.
- Demonstrates effective communication with patients, families, teams, & public.
Other Education & Training
- Basic Cardiac Life Support Certificate (BCLS)
Other Information
- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
- Registration with Canadian Association of Clinical Nutrition
- Working knowledge of current software programs
