Clinical Care Specialist (LPN)
About the role
Position Summary Granddaughters Home Care is seeking a Licensed Practical Nurse (LPN) with advanced clinical experience and leadership capabilities to join our team as Clinical Care Specialist. In this role, you will provide clinical oversight, support new and existing care staff, ensure quality standards are met across service lines, and participate in strategic care planning. You will play a key role in leading clinical initiatives, supporting training and mentorship. This position offers opportunities for advancement and professional growth for the right candidate. Key Responsibilities (evolving with the role) Provide clinical leadership, mentorship, and ongoing support to frontline care staff. Conduct client assessments and contribute to the development and oversight of individualized care plans. Lead onboarding, training, and skills development for caregivers (including personal care and advanced foot care). Act as the primary clinical escalation point for complex care situations and incidents. Provide direct clinical support as needed in the community, particularly for complex cases or first visits. Serve as a clinical escalation point for care teams and collaborate with the CEO and Care Experience Manager on incident response and care concerns. Support Occupational Health & Safety (OH&S) initiatives and compliance. Maintain clear and timely documentation in Alayacare. Contribute to program development and continuous improvement of clinical practices and service offerings. Qualifications Active and unrestricted LPN license in Alberta. Advanced Foot Care Certification (FCN) is considered an asset. Minimum (2) years' experience in a clinical leadership, supervisory, or advanced practice role. Minimum two (2) years of experience providing Home Care and clinical support to seniors living with dementia and other cognitive impairments. Strong mentoring, coaching, and interpersonal communication skills. Experience with client assessments, care planning, and clinical documentation. Knowledge of community home care best practices and healthcare regulations. High attention to detail, sound clinical judgment, and the ability to thrive in a fast-paced, flexible environment.
