About the role

This innovative facility integrates data and technology into patient assessment, care delivery, and communication, while providing specialized, person-centred support for individuals who no longer require hospital-level treatment but need additional care to safely return home or transition back into their community. Reporting to the Transitional Care Nurse, the TCA is integral in supporting patients and families meet the goal of returning to home/community by being engaged in interprofessional collaboration, using effective communication strategies, and mutual support.

About the Opportunity

  • Recognizes and reports a situation or change in patient circumstances, condition or behavior which poses a threat to the patient’s health, safety, security and well-being.

  • Assists patients with hygiene needs and daily living activities including bathing, dressing, oral care, skin care, positioning in bed, and transferring to and from bed to chair.

  • Assists with patient’s special needs, e.g. eye care, colostomy, and catheter.

  • Assists in restorative care e.g. walking, range of motion.

  • Provides mealtime assistance when required.

  • Documents according to facility policy e.g. Daily Patient Care Record, patient progress notes, initiating medication incident reports, servery documentation and audits, incident reports etc.

  • Communicates and reports pertinent and relevant information and/or observations to the Supervisor.

  • Completes rounds and corresponding documentation as per facility policy

  • Assists with the orientation and training of new employees

  • Additional duties where required

About You

  • In addition to placing high value on continuous improvement, collaboration and accountability, you bring:
  • You have a valid Continuing Care Certification or equivalent according to the CCA Entry to Practice policy;
  • You are registered in Nova Scotia’s CCA registry as per the Continuing Care Assistants Registry Act
  • You hold a valid CPR/Frist Aid certification. (required)
  • Proof of valid Basic Food Safety certification is required
  • Knowledge of infection prevention and control practices
  • Knowledge of principles of personal care including bathing, oral hygiene
  • Ability to utilize technology (ie Electronic Health Care Records)
  • Completion of certain certifications, such as Dementia: Understanding the Journey and Back in Form Lift & Transfer Program, considered an asset
  • You can work a schedule of flexible hours and shifts inclusive of days, nights, weekends, and holidays,
  • Previous experience supporting the care of vulnerable populations, with complex social situations, who are transitioning across health care settings, with a goal of home first
