About the role

Working as part of the Indigenous interprofessional primary health care team, the Nurse Practitioner will provide trauma-informed, culturally safe primary health care to full scope with a strong emphasis on health promotion and disease prevention for Indigenous patients. The Nurse Practitioner will assess, diagnose, treat and monitor a wide range of health problems while engaging patients as full partners in their care plan. Working under a guiding value of wholism, the Nurse Practitioner will work collaboratively with an interprofessional team including Traditional Healing, Social Work and Counselling, Nursing, Community Health Professionals, and Mental Health and Addictions Professionals to support physical, emotional, mental, and spiritual well-being for clients. The Nurse Practitioner brings advanced skills in primary care services to the table to support Indigenous communities. Working in a clinic space and in the community, the Nurse Practitioner will meet patients “where-they-are-at” as part of our trauma-informed care delivery model. This role is assigned to a specific base location with some regional work as required. 

What You Bring: 

  • Current registration and in good standing with the College of Nurses of Ontario as an RN (Extended Class) is required.  

  • Bachelors Degree in Nursing from a recognized University and Certificate as a Primary Health Care Nurse Practitioner OR Master’s degree in Nursing, Nurse Practitioner Program preferred. 

  • Membership in professional associations such as RNAO and/or NPAO is considered an asset. 

  • 2+ Experience working as an NP in a primary health care or community setting is considered an asset. 

  • Experience working with Indigenous patients and communities. 

  • Knowledge and understanding of Indigenous culture and protocols and willingness to work in community settings is an asset. 

  • Demonstrates a commitment to learning and openness to integrate cultural approaches to wellness into patient care  

  • Understanding and knowledge of a trauma-informed approach to health car delivery  

  • Ability to work well under pressure and multi-task without supervision. 

  • Ability to work independently as well as collaboratively with regulated health professionals and maintain a positive team environment. 

  • Ability to work in a manner that exemplifies the core values of compassion, respect, accountability, excellence and safety. 

  • Ability to provide trauma-informed care, while empowering patient driven care 

  • Experience with various computer applications including documenting in an electronic medical record (EMR) and other software related to healthcare programs (ie. PS suites, Microsoft 365) is considered an asset.
