Diabetes Educator and Primary Care Clinical Dietitian
Contract: Maternity Leave (16 months)
Full Time (1.0FTE): 3 days (0.6) in SUNDEC 2 days (0.4) in SAFHT
Sunnybrook Academic Family Health Team (SAFHT) and the Sunnybrook Diabetes Education Centre (SUNDEC) are looking for a highly motivated and self-directed Registered Dietitian to join our dynamic interprofessional team. This is a dual role, participating in both SUNDEC, serving a complex population of patients with diabetes, as well as at the SAFHT in primary care. Care for patients with diabetes will include Type 1 and Type 2 diabetes, with many requiring insulin. Care for primary care patients at SAFHT will include helping support their cardiovascular health, metabolic management, digestive system diseases and pediatric nutritional issues.
Job Summary

Conduct nutrition assessment of patients with incorporation of food habits, preferences, socio-economic and cultural backgrounds, psychological factors and medical profile

Provide medical nutrition therapy for patients based on assessment and plan, with consideration to literacy, language, cultural, and psycho-social factors

Educate and advise patients on self-management skills in individual and group setting focused on diabetes, including education and recommendations on use of technology to support care (glucose sensors, glucometers, uploads and reports)

Record assessments and plan in detailed reports of encounters with EMR (PSS)

Work with patients and inter-professional team members to develop and implement primary care coordinated care plans

Facilitate and develop group and community outreach education sessions

Ensure practice is consistent with professional standards as outlined by the College of Dietitians of Ontario and organizational policies

Demonstrate a commitment to professional lifelong learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession

Contributes to the academic mandate of the organization as an educator, resource or support for staff, students and members of the academic and professional community

Participate in Quality Improvement, and program development initiatives

Perform other duties as assigned
Qualifications:
- Graduated from an accredited or recognized program with a Bachelor of Science in Human Nutrition or equivalent
- Current registration in good standing with the College of Dietitians of Ontario (CDO)
- Certified Diabetes Educator Certificate (CDE) preferred
- At least 2 years of relevant experience as a Registered Dietitian working with patients with diabetes and in primary care and/or community-based care
- Experience working with complex patients with diabetes, including but not limited to use of multiple daily dose injections of insulin and insulin pump therapy
- Knowledge and ability to apply adult learning principles
- Experience in planning and implementing health promotion programs and activities
- Familiarity with client-centered and self-management approaches to chronic disease prevention and management
- Current CPR/BLS certification
- Ability to work with a high degree of autonomy and on inter-disciplinary teams
- Knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on diet and nutrition
- Proven flexibility and adaptability in a fast-paced environment
- Excellent time management, organizational and prioritization skills
- Excellent communication skills (verbal and written), critical thinking, and interpersonal skills
