Position: Supportive Housing Case Manager
Term: Full Time, Permanent
Community: Happy Valley-Goose Bay, NL

Responsibilities
Responsible for case management of individuals with multiple and complex needs and/or at risk of homelessness in the Upper Lake Melville area. Using an Intensive Case Management and Housing First Approach, provide assessment, counselling and intervention, as well as support in life skill development to address the multiple and complex bio/psycho/social/functional needs of individuals needing supportive housing Coordinate government and community services, e.g. NL Health Services (Mental Health Team – including Psychiatrist, Home Care and medical staff) and Justice (Labrador Correctional Centre, Probation, Parole, etc.) and other agencies and groups as part of a multidisciplinary team Meet as required with supported individuals and establish and monitor individual support plans to address any challenges and encourage continual progress in all life areas Provide outreach and mobile crisis intervention Continuously explore best practices/creative solutions to housing and supportive services for individuals with multiple challenges
Qualifications
Minimum Bachelor’s Degree in Social Work (Master’s preferred) or other behavioural science with current registration with NLASW Minimum 2 years case management and counselling experience On call and personal use of vehicle will be required
Assets
Bilingual in Inuktitut and English
