UHN’s vision is to build A Healthier World and it’s only because of the talented and dedicated people who work here that we are continually bringing that vision closer to reality. www.uhn.ca Job Description Union: Non-union Number of vacancies: 1 New or Replacement Position: Replacement Site: Toronto Western Hospital Department: Regional Stroke Program Reports to: Director, Regional Stroke Program Salary Range: $43.47 - $54.34 per hour Hours: 37.5 hours per week Status: Temporary Full-Time Closing Date: July 14, 2026 Position Summary Reporting to the Regional Director of the Toronto West Stroke Network, the eStroke Rehab and Referral Coordinator oversees the eStroke Rehab Referral pathway in the RM&R system. Data collected from the system is continually analyzed to monitor performance, system flow and efficiencies. Data facilitates the identification of quality improvement initiatives to enhance timely access to stroke rehabilitation programs in Toronto, and improve patient experience outcomes Duties Maintaining responsibility for the overall administration/management of the referral system Referral system education/training Identifying/facilitating/troubleshooting system database/process upgrades Sustain engagement of acute and rehab partners Supporting emerging data and best practice driven system quality improvement initiatives related to rehabilitation access/flow as appropriate Participating member of the Ontario Rehabilitation Coordinators Group working to facilitate the implementation of best practices stroke care within local organizations, within the region and across the continuum of care. Qualifications Completion of a Master’s degree in Health Education or Administration related program preferred Regulated Healthcare Professional, minimum 4 years experience Clinical experience essential; rehabilitation experience preferred. Knowledge of Rehabilitation system in Ontario. Familiarity and evidence of certification with common outcome measures (FIM or AlphaFIM ® required) Knowledge of the Quality of Care outcome measures, stroke outcome measures. Strong meeting organization and facilitation skills. Computer literacy at an advanced level (MS Office, Excel an asset) Compliance with confidentiality agreements History of related project work preferred Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand.
