The Clinical Care Coordinator will act as a resource and advocate for refugees while establishing partnerships to support a coordinated access pathway for care. They will also perform physical assessments, provide health education, and contribute to inter-professional team planning and documentation.

The Registered Nurse will perform clinical assessments, develop client-focused treatment plans, and provide health education for patients with diabetes. They will work collaboratively within an inter-professional team to ensure optimal client care and participate in program evaluation and quality improvement.

The Registered Practical Nurse will perform physical assessments, provide health education, and offer counseling to clients within the Refugee Health Program. They are also responsible for maintaining clinical areas, documenting patient progress, and participating in inter-professional case conferences.
