Role Description This is a part-time, on-site Nurse Practitioner role located in Toronto, ON. The Nurse Practitioner will provide direct patient care, including comprehensive assessments, diagnosis, and development of integrative treatment plans aligned with functional medicine principles. Day-to-day responsibilities include conducting in-person consultations, ordering and interpreting laboratory and diagnostic tests, prescribing medications where appropriate, and recommending evidence-based lifestyle and nutrition interventions. The role involves collaborating with physicians and other healthcare professionals, documenting care in the medical record, and participating in case reviews and care planning meetings. The Nurse Practitioner will also provide patient education, support follow-up care, and contribute to quality improvement and clinic protocols to enhance patient outcomes. Qualifications Current Nurse Practitioner license (or equivalent designation) in good standing with the appropriate regulatory body in Ontario. Clinical experience in primary care, internal medicine, family medicine, or a related patient-facing setting. Demonstrated interest or training in integrative and functional medicine, lifestyle medicine, or holistic care. Strong assessment, diagnostic reasoning, and clinical decision-making abilities. Excellent communication and interpersonal skills, with the ability to build trusting relationships with diverse patients. Ability to work collaboratively in an interdisciplinary team and contribute to shared care plans. Proficiency with electronic medical records and basic digital tools used in clinical practice. Strong organizational skills, attention to detail, and ability to manage a part-time patient schedule effectively. Commitment to continuous learning, evidence-based practice, and maintaining relevant continuing education. Experience in an outpatient clinic setting and familiarity with the Toronto healthcare landscape are assets.
