Deliver comprehensive primary care across the lifespan, serving as the Most Responsible Provider for a patient panel and supporting same-day appointments. Coordinate interdisciplinary care, lead quality improvement and program initiatives, and provide patient and family education focused on health promotion and disease prevention.

The Registered Practical Nurse will work within interdisciplinary teams to provide comprehensive, patient-centered care. This includes facilitating and delivering both group and individual patient education.
