Job Description You will be responsible for providing high-quality nursing care to patients in the community. You have clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management. Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of the health care team Ensure that nursing care provided to our patients is based on best practices to achieve successful outcomes Continually assess, anticipate, identify and respond to new and changing needs of our patients Travel within your region between patients’ homes Qualifications Minimum two years nursing experience (Community Nursing experience is ideal) Registered and in good standing with the College of Nurses of Ontario (CNO) Ability to work independently in the community Proficiency in English, both verbal and written required (additional languages an asset) Valid Ontario driver's license and reliable vehicle Experience with infusion therapy, mental health, palliative care, and wound care Excellent patient assessment and teaching skill Pay per visit model (compensation is based on experience. Requirements
