The Sherwood Park – Strathcona County Primary Care Network works as a team with family doctors and other healthcare professionals to advance the health and well-being of the communities we serve. We are one of the 40 Primary Care Networks (PCN) in Alberta. PCNs strive to improve integration of patient care, disease prevention and raising the general health of Albertans, chronic disease management, capacity and patient access, and the overall health and wellness of patients.

The Licensed Practical Nurse will provide direct clinical care, including immunizations, wound care, and patient assessments within a family medicine clinic. They will collaborate with an interdisciplinary team to develop care plans, provide health education, and coordinate patient resources.
