Les programmes et services au Centre sont offerts par une équipe interdisciplinaire. La personne est responsable de fournir des services professionnels de promotion de la santé dans la communauté. Elle fournit des interventions et des traitements dans la clinique ou dans la communauté en collaboration avec les clients pour assurer la continuité de leurs soins. RESPONSABILITÉS Fournir des services professionnels de soins primaires intégrés en misant sur la promotion de la santé et la prévention de la maladie et des blessures. Évaluer, gérer et consulter avec l’équipe interdisciplinaire le dossier du client en étroite collaboration avec le client ou la famille. Documenter les soins cliniques des clients dans les plus brefs délais. Soutenir l’exécution des plans de soins en collaboration avec l’équipe interdisciplinaire pour gérer les besoins de santé complexes de la clientèle. Consulter les médecins, les infirmières praticiennes ou infirmiers praticiens et les autres membres de l’équipe interdisciplinaire pour diverses situations relatives à la clientèle. Trier les demandes de renseignements (par téléphone, en personne ou par courrier électronique) et y répondre; déterminer le niveau d’urgence des demandes et assurer les suivis. Maintenir des liens avec les groupes communautaires et les autres organismes de services de santé en participant à des réunions et à des événements communautaires. Offrir un service à la clientèle tout en se conformant avec la Loi sur les infirmières, et aux actes réservés de l’Ordre des infirmières et des infirmiers du Manitoba (College of Registered Nurses of Manitoba). Adopter une pratique fondée sur l’expérience clinique avec les personnes et les communautés. Participer et contribuer aux séances ou aux ateliers d’éducation en groupe relatifs à son service. Faire preuve de diligence, de confidentialité et de loyauté dans ses rapports avec son employeur. Assurer une communication efficace au sein de son service et entre les services. EXIGNCE DU POSTE Détenir un baccalauréat en sciences infirmières reconnu par la province du Manitoba. Détenir une certification de compétence professionnelle décernée par l’Ordre des infirmières et des infirmiers du Manitoba. Être membre en règle de l’Ordre des infirmières et des infirmiers du Manitoba. Posséder de 3 à 5 années d’expérience en milieu communautaire, en santé publique ou en éducation et promotion de la santé. Posséder une connaissance approfondie de la promotion de la santé, des concepts du développement et des ressources communautaires. Être capable de répondre aux besoins d’une population vulnérable. Être physiquement capable de soulever des poids moyens, de demeurer assis pendant de longues périodes et de se promener au courant de la journée. Être physiquement capable d’exercer les fonctions du poste dans diverses conditions météorologiques ou environnementales. Maîtriser les deux langues officielles du Canada, tant à l’oral qu’à l’écrit. Faire preuve de pensée critique, novatrice et proactive pour répondre aux besoins de la clientèle et de ses collègues. Posséder d’excellentes compétences en communication, en résolution de conflits et en relations interpersonnelles. Entretenir de solides relations de travail à l’interne et à l’externe. Programs and services at the Centre are provided by an interdisciplinary team. The incumbent is responsible for providing professional health promotion services in the community. The incumbent provides interventions and treatments in the clinic or in the community in conjunction with clients to ensure continuity of care RESPONSIBILITIES Provide professional, integrated primary care services focusing on health promotion and disease and injury prevention. Consult with the interdisciplinary team to evaluate and manage the client’s file in conjunction with the client or family. Document clients’ clinical care as promptly as possible. Support the implementation of care plans in conjunction with the interdisciplinary team to manage the complex health needs of clients. Consult with physicians, nurse practitioners and other members of the interdisciplinary team with respect to various client- related situations. Sort and respond to telephone, in person or email information requests; determine how urgent request are, and follow up as required. Maintain ties with community groups and other health service organizations by participating in community meetings and events. Provide customer service in accordance with the Registered Nurses Act and Acts governing the College of Registered Nurses of Manitoba. Adopt an evidence-based practice with people and communities. Participate and contribute to group education sessions or workshops relating to his/her department. Demonstrate diligence, confidentiality and loyalty in dealings with employer. Ensure effective communication within his/her department and between departments. Know and comply with all organizational policies, procedures and standards, as well as legislative, regulatory, professional and accreditation requirements. Keep up to date on appropriate handling and transportation of hazardous products; comply with the standards outlined in the Infection Control Manual, Emergency Preparedness Manual and the Workplace Hazardous Materials Information System (WHMIS). JOB REQUIREMENTS Bachelor of Science in Nursing recognized by the province of Manitoba. Certificate of Practice issued by the College of Registered Nurses of Manitoba. Active registration with the College of Registered Nurses of Manitoba. Three to five years of experience in a community setting, public health or health education and promotion. Extensive knowledge of health promotion, development concepts and community resources. Be able to meet the needs of a vulnerable population. Must be able to do moderate lifting, sit for long periods of time, and walk during the day. Be physically able to perform the duties of the position in a variety of weather or environmental conditions. Have very good oral and written command of both of Canada’s official languages. Requirements
