About Rapids Family Health Team The Rapids Family Health Team is funded by Ontario Health and works in partnership with Family Health Organization (FHO) physicians to support patients in achieving better health outcomes. We focus on both preventing future health concerns and supporting the management of existing conditions through a collaborative, patient-centered approach to care. Patients referred by their FHO physician can access a wide range of allied health services, including: • Diabetes education and management (insulin teaching, adjustments, and support) • Smoking and vaping cessation counselling • Nutrition and dietary support with a Registered Dietitian • Occupational Therapy services • Medication support with a Registered Pharmacist • Cognitive screening • Respiratory assessments and education (asthma, COPD, CPAP support, and more) • Well Woman Clinic services (Pap tests and clinical breast exams) • Mental health support through Registered Social Workers and Psychologists • Congestive Heart Failure support program We are committed to providing accessible, team-based care with minimal wait times and no cost for patients of FHO physicians. To ensure inclusive communication, free medical translation services are available in over 240 languages across all locations. The Occupational Therapist will provide assessments and interventions for falls prevention, balance, and home safety for patients referred by physicians. They will also facilitate specialized group programs, such as Boosting Balance, and prescribe assistive devices.

The Nurse Practitioner will perform comprehensive patient assessments, formulate medical diagnoses, and coordinate diagnostic tests within acute and episodic clinics. They are also responsible for maintaining accurate patient records and participating in the development of health promotion programs.
