Chronic Care - Home Health Nurse

In this role, you will provide patient-focused, safe, holistic nursing care and work collaboratively with our team members to help patients maintain physical, mental, and emotional health. You will take the time to understand the needs of each patient in order to provide a customized treatment plan that provides lasting relief for them and eventually helps their families as well.

This is a casual position for Nurses who are looking for afternoon or evening shifts NO travel is required between patients’ homes We offer up to full time hours. Flexible shift start and end times.

What you will need to be successful

• You are registered as a member in good standing with the College of Nurses of Ontario
• You are able to provide proof of your liability insurance, current CPR and First Aid certification, and police check with a clear vulnerable sector screening
• Ability to work every other weekend.
• Strong critical thinking skills
• LEAP Training is considered an asset
• Strong verbal and written English communication skills
• The ability to communicate in French or other languages is considered an asset
• All team members are required to be fully vaccinated against COVID-19 and provide proof of vaccination upon hire or provide a valid medical exemption
• A valid driver’s license and a reliable insured vehicle is considered an asset
