Nurse Practitioner – Complex Care
Are you a qualified Nurse Practitioner with acute care, complex care or long-term care experience, looking to share your expertise? If so, Niagara Health's Complex Care department has an exciting full-time position for you at our Douglas Memorial Site.
Reporting directly to the Site Director, and working in collaboration with the Niagara Health Family Medicine Program, you will be responsible for the ongoing assessment, planning, management and treatment of patients within the complex care unit in collaboration with the Douglas Memorial Physicians. As a Nurse Practitioner (NP) in Complex Care, you will provide leadership and promote excellence in patient-centred care through expert practice, education, consultation and research. This is an exciting opportunity to provide care with an emphasis on wellness, prevention and disease management, not only through direct patient care, but also through health promotion and education, outreach, advocacy and community development, as needed.
While your home site will be the Douglas Memorial, you will be rotating to other Niagara Health sites, as required.
