Clinical Coordinator AFTER HOURS Team Lead

Job Summary

Reporting to the Clinical Coordinator Supervisor After Hours, the Clinical Coordinator Team Lead After Hours is responsible for planning, organizing and supporting the daily operations of the After Hours Clinical Coordination team to ensure clinical service delivery meets or exceeds standards as set by CBI Home Health external stakeholders. The Clinical Coordinator Team Lead After Hours will act as a resource for the After Hours Coordination and After Hours Clinical Coordination teams.


  • Active Registration with the College of Licensed Practical Nurse of Alberta
  • Some experience related to community care preferred
  • Leadership experience is considered to be an asset
  • Must have the ability to work from home and must have a complete home office setup, including access to high speed internet and a personal home computer
  • Available to work shift work, nights, weekends and statutory holidays
  • Proficiency in Microsoft Office Suite, and the ability to learn new programs specific to the business

Essential Functions

  • Provide support and resources to the Clinical Coordination After Hours Team
  • Mentors, coaches and works collaboratively within the Clinical team to support new members including Care Coordination team
  • Ensure that the field employee understands their role, care to be provided and is competent in providing required care
  • Use the scheduling software provided to its fullest capacity to create efficiencies in all aspects of the coordinators role as per specified standards including:
  • Manage requests for service and coordinate assignment of care
  • Manage changes to service provision and updates to client Care Plans
  • Maintain electronic client and field employee’s files where required
  • Document professionally, appropriately, and in a timely manner Interact with clients to:
    • Identify their needs and preferences
    • Inform them of whom the field employee will be
    • Inform them of day/time service will be provided
    • Ensure that they are satisfied with the quality of care being provided
    • Resolve concerns
  • Document client concerns for follow up with referring agencies
  • Knows and adheres to the formal and informal departmental goals, standards and policies and procedures
  • Work collaboratively with Field Supervisors as necessary and document for follow up as required
  • Participate in office meetings and additional training as required
  • Work with an awareness of health and safety and cooperate towards improved health and safety conditions at work.

Physical Requirements

  • Sitting in a normal seated position for extended periods of time
  • Reaching by extending hand(s) or arm(s) in any direction
  • Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard
  • Communication skills using the spoken word
  • Ability to see within normal parameters
  • Ability to hear within normal range
  • Ability to move about

Hours: Rotation: Monday to Friday 2pm-10pm (one day off during the week Tue/Wed/Thu)

+ Every Other Weekend **alternating hours** 7am-3pm and 2pm-10pm
