As a member of a primary health care (PHC) Diabetic education Centre team, the PHC Dietitian provides a range of PHC services. These services include, but may not be limited to, health promotion, illness prevention, screening, treatment and monitoring of people living with diabetes. Expertise may also be required to support people living with other chronic health problems, community development and advocacy. The incumbent will be required to collaborate with others to increase community capacity as an educator, researcher and clinical practitioner. These services are provided within a holistic, client/family centred care framework. The incumbent will provide health assessment, interventions and appropriate follow up in accordance with acts, regulations, policies, standards, guidelines, mission and objectives of the Registered Dietitians of Canada, the Nova Scotia Dietetic Association, the Department of Health and Wellness - Province of Nova Scotia and the Nova Scotia Health. ABOUT YOU We would love to hear from you if you have the following: * Bachelor of Science in Human Nutrition at a recognized university * Dietetic internship (a 1 year integrated or post graduate program) * Current Membership in Nova Scotia Dietetic Association required * Annual continuing education to maintain Nova Scotia license * Two years clinical experience preferred * Membership in Dietitians of Canada preferred * Current CPR/BLS certification is required * Designation as a Certified Diabetes Educator preferred * Experience with group education * Demonstrated time management and organizational skills * Familiarity with principles of Motivational Interviewing * Exhibits creative problem-solving ability; Ability to work collaboratively with other disciplines and committees to achieve excellence in patient care * Computer proficiency in MS Office (Word, PowerPoint), email and internet * Ability to work well independently as well as within a multidisciplinary team environment * Excellent verbal and written communication skills * Competencies in other languages an asset; French preferred Please ensure your resume is up to date and includes all relevant education, experience, training, and certifications. Requirements
Candidates must hold a Bachelor of Science in Human Nutrition and have completed a dietetic internship with current Nova Scotia Dietetic Association membership. Two years of clinical experience and CPR/BLS certification are required, with Certified Diabetes Educator designation preferred.
