Deliver specialist stroke rehabilitation to people across Angus in community settings, including patients’ homes, workplaces, local communities, and outpatient appointments. Work collaboratively with physiotherapists, speech and language therapists, inpatient rehabilitation teams, the stroke liaison team, and other partners to support patients returning home and continuing rehabilitation.

Lead and support the community occupational therapy stroke team, combining clinical practice with professional leadership, staff development, and service improvement. Collaborate across health and social care settings to deliver person-centred stroke rehabilitation, including early supported discharge, and help people maintain independence in their communities.
