The Kemptville Nurse Practitioner-Led Clinic is a primary care organization dedicated to improving access to high-quality, collaborative, team-based healthcare for the residents of North Grenville and the surrounding communities. As a key partner of the Kemptville Primary Care Hub, the KNPLC works alongside family physicians, healthcare providers, community organizations, and system partners to strengthen access to integrated primary care and improve the health and well-being of our community. Our interdisciplinary team of Nurse Practitioners and allied health professionals provides comprehensive, patient-centered care focused on prevention, health promotion, chronic disease management, and helping individuals navigate the healthcare system. Through collaboration, innovation, and strong community partnerships, we are working to ensure people receive the right care, at the right time, in the right place. Provide comprehensive primary care across the lifespan, including assessment, diagnosis, treatment, prescribing, prevention, chronic disease management, and coordination of referrals and follow-up. Collaborate with an interdisciplinary team, mentor learners and colleagues, and contribute to quality improvement, clinical program development, and accurate EMR documentation.
